Transitional Housing Program 

Helping foster youth learn how to live independently.

 Transitional Housing (THP+FC) is a licensed placement for youth, ages 18-21 who are participating in extended foster care made available by Assembly Bill 12. THP+FC provides housing and comprehensive support services to youth.

In this program, youth 18-21 live independently while still receiving services from Parents by Choice. Some of the services received include housing, case management, vocational support, support to attend college, mentorship and more. Our team of professional staff and volunteers work with the youth to help them transition into adulthood.

Who is eligible for this program?
  • Youth age 18 - 21 years old
  • Former foster youth
  • Youth can be from any county, but currently reside in San Joaquin or Stanislaus County

Parents by Choice provides community meetings for the youth to learn and improve skills such as filing taxes, how to manage money, cook and much more. We encourage the youth to create a support system with the other youth, staff, volunteers and community members. 

Our goal is to make sure each youth that leaves our agency can live successfully as an independent adult.

Parents by Choice serves youth in San Joaquin County and Stanislaus County.
